Does it work on iPhone?
No, not directly. The Premium Spotify APK is the official Spotify Android app with the subscription check patched out and the ad code removed, and APK is an Android-only format. An iPhone cannot open or install an APK at all, so this exact file will never run on iOS. You still have a working path to the same result, it just uses a different file type.
On iPhone you install a patched Spotify++ IPA instead. You sideload that IPA onto your device with AltStore or Sideloadly running on a Windows or Mac computer, and you sign it with a free Apple ID. The catch with a free Apple ID is the certificate. It lasts 7 days, so the app stops opening after a week and you re-sign it to get another 7 days. AltStore can refresh it automatically over Wi-Fi if you leave its helper running, which saves you reconnecting by cable every time. The end result matches the Android mod closely. Two things to weigh before you start. First, using any patched Spotify client breaks Spotify’s Terms of Service, so a small ban risk exists on iOS exactly as it does on Android. Sign in with a spare free account and keep your main account on the official app. Second, there is no App Store and no automatic update, so when a newer build lands you re-sideload it manually.
